Technical Assistance:
1. Reviewed requirements for keeping products labeled "Keep out of reach of children" locked when not in use.

Comments:
Conducted an unannounced monitoring inspection. Observed 20 children + 2 direct-care staff. Ratios and supervision were in compliance. Staff were actively involved with the children and activities. Children were observed having active outdoor play, washing hands, eating snack, and doing homework. The activity area was found to be clean and sufficiently supplied with equipment for the children. Several children approached the inspector to discuss their desire for different activities for the older children in the group and their previous efforts to communicate this with staff. Snack served: chocolate chip graham crackers + milk. No medications reported to be on-site. Required documents were posted. Record keeping was organized, though violations found. Inspection completed 3:20-4:15pm.

Violations:
Standard #: 22VAC40-185-50-A
Description: Based on observations made, it was determined that confidential information about children was not treated in a confidential manner. Evidence: The Supplemental Information page of the 9/24/13 inspection report, which identified a child by name, was posted. This page of the report is not to be posted and it states on the document, "Information found on the Supplemental Information page is confidential and this document is not to be posted in the facility."

Plan of Correction: supplemental information page will not be posted in the future [sic]

Standard #: 22VAC40-185-280-B
Description: Based on observations made, it was determined that hazardous substances were not kept in a locked place. Evidence: A can of spray disinfectant was in an unlocked cabinet located in the cafeteria.

Plan of Correction: all hazardous substances will be placed out of reach in a locked container [sic]

Standard #: 22VAC40-185-550-M
Description: Based on records reviewed, it was determined that 4 of 4 written injury reports did not include the following documentation. Evidence: 1. 4 of 4 reports did not include future actions to prevent recurrence. 2. 2 of 4 reports did not include the date and time the parent was notified. 3. 2 of 4 reports did not include how the parent was notified.

Plan of Correction: Staff will be given a training on proper sections to complete + all future incident reports will contain complete information.

Standard #: 22VAC40-80-120-E-2
Description: Based on observations made, it was determined that the most recently issued Violation Notice was not posted. Evidence: The report from 7/9/14 was not posted.
