Comments:
An unannounced monitoring inspection was conducted today. All groups are in ratio. 1 staff and 3 children's files were reviewed. See violation notice for missing information. Medications were reviewed. See violation notice for missing information. First aid supplies, emergency escape drills, accident/injury reports were all in compliance. Violations:
Standard #: 22VAC40-185-40-G
Description: Based upon a review of files, one child's file is incomplete. Evidence: Child A does not have documentation available to show that an original Proof of Birth document was observed.

Plan of Correction: I will contact the parent and ask to have the document brought in. Children's files will be audited for complete information.

Standard #: 22VAC40-185-280-B
Description: Based upon direct observation, one container of bleach water was not stored locked when not in use.

Plan of Correction: Locked up during the inspection. I will remind staff that the liquids must be locked unless they are in use.

Standard #: 22VAC40-185-510-E
Description: Based upon a review of medications, one Over-the-counter medicine was not labeled to show ownership.

Plan of Correction: Medicine will be labeled.

Standard #: 22VAC40-185-560-G
Description: Based upon direct observation, one food container from home was labeled to show ownership; however, it was not dated.

Plan of Correction: The parents will be informed that the container must be labeled and dated.
