63.2 Facilities and Programs..

Technical Assistance:
Staff files are not reviewed during on-site inspections. This is completed during an annual administrative file review. Child #5 has an epi-pen on site, which expired March 2015.

Comments:
An unannounced monitoring inspection was initiated today at 3:15pm. There were 48 children under the supervision of 3 staff. During the inspection, children were observed working on their homework, coloring, reading books, enjoying free play and cleaning up for afternoon snack. The children were offered graham crackers and milk. There was a substitute Site Supervisor present today. The findings of the inspection were reviewed with her. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on file review, 1 out of 4 children's files does not have the results of a physical examination on file. Evidence 1. Child #1 does not have a physical examination on file.

Plan of Correction: We will ask the parent to bring this into the center to keep on file.

Standard #: 22VAC40-185-510-C
Description: Based on observations, the center did not follow all procedures for administering medications. Evidence 1. Child #5 requires Ventolin as an emergency medications, while in care at the center. This medication's expiration date was March 2015.

Plan of Correction: We will ask the parent to replace this.
