Comments:
An unannounced renewal study was conducted from 10:15 am - 1:00 pm. There were 69 children in care being supervised by 11 staff members. The children were observed in structured center play, circle time activities, and outdoor play. Lunch served today is beef crumbles, pasta, green beans, pears, and milk. The center is equipped with a wide variety of age/stage appropriate supplies, activities, and equipment. The most recent health inspection was conducted 11/12/2014 and the most recent fire inspection was conducted 11/6/2014. The most recent fire drill was conducted 3/10/2015 and the most recent shelter in place drill was conducted 3/31/2015. The first aid kit, flashlight, and radio were observed and in compliance. Three busses were observed and in compliance. Seven staff records were reviewed. Ten children's records were reviewed and in compliance. Six medications were reviewed and in compliance. There are currently seven staff members who are MAT (Medication Administration Training) certified. Standard #: 22VAC40-185-500-A
Description: Based on observation of diapering it was determined that not all children's hands were washed after diapering. Evidence: The LI (Licensing Inspector) observed that a child receiving a diaper change did not have their hands washed after being diapered by staff A. The child's hands were washed after the LI reminded staff A regarding hand washing.

Plan of Correction: Staff A will be trained to wash hands after diapering.

Standard #: 22VAC40-185-500-B
Description: Based on observation of diapering it was determined that not all procedures were followed during the diapering process. Evidence: Staff A did not sanitize the diapering surface after cleaning.

Plan of Correction: Staff A will be trained to sanitize the diapering surface after cleaning.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on staff record review it was determined that not all staff received background checks before the end of 30 days of employment. Evidence: Staff A with a start date of 1/28/2015 did not have record of a Central Registry background check.

Plan of Correction: Administrator has mailed the Central Registry check however, the results have not been mailed to the facility. The administrator called central registry to check the status of the background check during the inspection. Staff A will not be permitted to return to work until the results have been given to the facility.

Standard #: 22VAC40-191-60-C-2
Description: Based on staff record review it was determined that staff were permitted to work after 30 days of employment without receiving record of central registry background check. Evidence: Staff A with a start date of 1/28/2015 was continuing to work at the facility without receiving documentation of central registry background check.

Plan of Correction: Based on staff record review it was determined that not all staff received background checks before the end of 30 days of employment. Evidence: Staff A with a start date of 1/28/2015 did not have record of a Central Registry background check.
