22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted today from 10:35am-12:00pm. There were 112 children (infants-5yrs.) directly supervised by 14 staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports and emergency supplies were inspected. Children were observed eating snack, playing outside, participating in a soccer program, and completing art work. There was an abundant supply of materials for the children. Standard #: 22VAC40-185-270-A Description: Based on direct observation, areas and equipment of the center were not maintained in a clean, safe, and operable condition. Evidence: Two tricycles located on the playground were observed without handle grips, exposing a hole in the handles, which poses as a finger entrapment hazard. Plan of Correction: We will have it taken care of.
Standard #: 22VAC40-185-280-B Description: Based on direct observation, hazardous substances were not kept in a locked place. Evidence: A spray bottle labeled "Bleach" and a can of Lysol spray were observed in unlocked cabinets in the Intermediates bathroom. Plan of Correction: Will be corrected immediately and will conduct a classroom licensing check.
Standard #: 22VAC40-185-290-3 Description: Based on direct observation, an electrical outlet did not have a protective cover. Evidence: An outlet located on the lower wall near the computer desk was observed without a protective cover in the AM Preschool classroom. Plan of Correction: We will fix it immediately and the teachers will get covers. Will conduct a classroom licensing check.
Standard #: 22VAC40-185-550-M Description: Based on record review, written records of children's injuries were incomplete. Evidence: 1. The time in which parents were notified of children's injuries was not observed documented on three of four injury records reviewed. 2. The method of how the parents were notified of children's injuries was not observed documented on two of four injury records reviewed. 3. The future action to prevent the recurrence of the injury was not observed documented on one of four injury records reviewed. Plan of Correction: Will review with the classrooms and will review procedures in the staff meeting next week.
