Technical Assistance:
Discussed with Director documentation of annual update required for children's files.

Comments:
An unannounced monitoring inspection was conducted today 04/21/2015 from 10:30am- 12:00 pm. Ratios and supervision were in compliance. Classes were observed participating in a story time activity, playing on the playground and eating snack. The inspector also observed a child being comforted in the office that was not feeling well. The physical plant, children's records, medications and emergency supplies were reviewed at the time of inspection. Areas of non- compliance are identified in this report. Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation and interview, hazardous materials were found not to be kept in a locked place using a safe locking method that prevents access by children. Evidence: In the preschool classroom, white out marked "keep out of reach of children" was sitting on a counter within reach of children in care.

Plan of Correction: The white out was locked during the inspection.

Standard #: 22VAC40-185-510-A
Description: Based upon record review, required authorization for medication was found to be expired. Evidence: Child A's medication Authorization for emergency Epi-pen expired as of 03/2015. The center had verified that all forms were renewed at the beginning of the school year and were all set with a year expiration date as needed. The authorization form obtained for child A was an older form set with a six month expiration date. All medications had been checked for current expiration dates and documented in the office.
