Technical Assistance:
Record Keeping was discussed with the director. Different pieces of the required documents were kept in different buildings on the main school's campus and several different people were responsible for individual items. It is recommended, documents required for the licensed child day center be kept in one area to help aid staff in overall organization. and that the persons designated as overseeing the licensed program are able to easily access the information.

Comments:
An unannounced monitoring inspection was conducted from 10:00am-12:30pm. Upon arrival there were 35 children present with 10 staff present. Children were observed eating snack and on the playground. A sample of 2 staff and 4 children's records were reviewed in addition to 4 medications.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on a record review the center failed to ensure all required information was kept for each staff person. Evidence: Staff #2 was listed in the record as a program leader, however there was no documentation that showed she met the requirements of the position.

Plan of Correction: Staff #2 has a degree. The director will ask Staff #2 to obtain her transcript so she can put it in the file.

Standard #: 22VAC40-185-290-3
Description: Based on observation the center failed to ensure al electrical outlets had protective covers. Evidence: In room 128 there was an electrical outlet located at the baseboard on the circle time wall that did not have a protective cover.

Plan of Correction: The director will ensure all electrical outlets have covers and that there are extras in each classroom.

Standard #: 22VAC40-185-560-F
Description: Based on observation the center failed to ensure all requirements were met when choosing to provide meals or snacks. Evidence: The children were observed eating pretzels for snack. The menu for that day indicated cheese sticks were to be served. The substituted food was not indicated.

Plan of Correction: The director will review policies for providing meals and snacks and will ensure what is on the menu is being served or the substitute food is indicated.
