Comments:
An unannounced monitoring inspection was conducted from 11:00 a.m. to 12:50 p.m. At the time of entrance the following adult to child ratios were observed: Classroom B - 18 children, ages 3 to 5 years, with 2 staff Classroom A - 18 children, ages 3 to 5 years, with 3 staff The records sample size consisted of 3 children?s records. Staff records are housed off site and will be reviewed in a separate inspection. Children were observed practicing hand writing and letter formation, dancing and singing to music, engaged in circle time discussion about phonics, the solar system, and washing hands and preparing for lunch. Restroom procedures, hand washing and table sanitizing were observed. Medication administration was not observed. Areas of non-compliance are identified on the violation notice. Thank you for your time during today's inspection. Please do not hesitate to contact me if I may be of assistance. Staff #1, the designated contact staff, provided verbal responses to the violations during the inspection, which were entered on to the violation notice by the Licensing inspector.

Violations:
Standard #: 22VAC40-185-510-A
Description: Based on review of accident reports, the licensee did not ensure that non prescription medication is only administered only with written authorization from the parent. Evidence: A written accident / injury report, for child #4, dated 4/13/15, was reviewed by the licensing inspector during the inspection. The written information on the form said that when a child fell and scraped her knees " TLC was given along with a cold compress and Triple Antibiotic Cream". When asked to review the authorization form for administering "Triple Antibiotic Cream, Staff #1 stated that there was not a written authorization completed, but that the parent had given verbal authorization over the telephone.

Plan of Correction: Staff #1 removed the Triple Antibiotic Cream from the supply closet and stated that she would dispose of it and remind staff that a minor cut or scrape can be cleaned, but not treated with ointments or other topical medications without authorization in writing from the child's parent.

Standard #: 22VAC40-185-560-C
Description: Based on a review of the posted schedule and interview, the licensee did not ensure that there is a period of at least 1hour but no more than three hours between each meal or snack unless there is a scheduled rest or sleep period for children between the meals and snacks. Evidence: The posted daily schedule showed Breakfast on the schedule from 8:15 a.m. to 8:45 a.m. and lunch from 12:00 to 12:30 p.m. There is more than 3 hours between 8:45 when breakfast ends and 12:00 when lunch begins. When asked if the schedule had been changed recently, staff #1 stated that it had been recently adjusted. When it was brought to staff attention that there was more than 3 hours between meals, staff stated that the actual schedule varies somewhat from what is written or planned and that breakfast normally continues later than scheduled anyway since there are often short delays in delivery.

Plan of Correction: Staff #1 stated that she will adjust the schedule today to have breakfast slightly later since this is frequently the case anyway. No more than 3 hours will lapse between each meal or snack unless children are resting or sleeping.
