Technical Assistance:
Inspector discussed 22 VAC 40-185-510-N and 22 VAC 40-185-60-A-6 with the Director during this inspection. Potential modification request was also discussed at this inspection.

Comments:
An unannounced monitoring visit and facility tour was conducted from 9:41 am-10:35am. At the time of inspectors' arrival there were 85 3-6 year old children in care with 6 staff members and 2 administrators. Children were observed participating in Montessori activities/instructional time in the classroom, transitioning between activities, hand washing and snack time. A sample of 7 children's records and 3 staff records were reviewed. Medications and injury reports were also reviewed. Areas of noncompliance were discussed with the Director during exit meeting at the conclusion of this inspection. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 22VAC40-185-70-A
Description: Based on record review and interview, it was determined that the center did not ensure that each staff record contained all of the required components. Evidence: 1.The record for Staff #2(hired 8/26/14) did not contain written information to demonstrate that the individual possessed the orientation training and two references required by the job position, Aide. 2. The record for Staff #3(hired 8/28/14) did not contain written information to demonstrate that the individual possessed the education and orientation training required by the job position, Program Leader.

Plan of Correction: The provider?s responses for the ?Description of Action To Be Taken? were not received as of 5/2/2015 and will not appear on this Violation Notice.
