22VAC40-191 Background Checks (22VAC40-191)
An unannounced renewal inspection was conducted today from 10:40am - 1:10pm. There were 60 children (infants-5yrs.) directly supervised by 12 staff. The physical plant, 5 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books and playing outside. Diapering and hand-washing procedures were also observed. There was an abundant supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Standard #: 22VAC40-185-60-A Description: Based on review, the facility failed to ensure that all children's records had the required information. Evidence: 1) Reviewed 5 children's records and found that CH3 and CH5 did not have documentation of emergency contact addresses. 2) Reviewed 5 children's records and found that CH1 did not have documentation of the parents annually reviewing, and updating as necessary, the emergency contact information. Plan of Correction: Staff will obtain documentation today.
Standard #: 22VAC40-185-280-B Description: Based on observation, the facility failed to ensure that all hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: Observed hand sanitizer in an unlocked cabinet in the preschool classroom. Plan of Correction: Corrected during the inspection. Staff placed the hand sanitizer in a locked cabinet.
Standard #: 22VAC40-185-500-B Description: Based on observation, the facility failed to maintain the diapering surface so that it was nonabsorbent. Evidence: Observed a tear in the diaper changing mat in the Infant One classroom. Plan of Correction: Corrected during the inspection. Staff replaced the diaper changing mat with a new one.
Standard #: 22VAC40-185-550-B Description: Based on review, the facility failed to ensure that they had an emergency preparedness plan with the required procedural components. Evidence: Reviewed the emergency preparedness plan and found that they only had written documentation of the evacuation drills. Plan of Correction: Staff will create and maintain a written emergency preparedness plan.
Standard #: 22VAC40-185-550-M Description: Based on review, the facility failed to ensure that injury/accident reports are documented with the required information. Evidence: 1) Reviewed 11 injury/accident reports and found that 10 did not have documentation of any future action to prevent recurrence. 2) Reviewed 11 injury/accident reports and found that 1 did not have documentation of the time parents were notified. Plan of Correction: Staff member will create a memo for all staff, and remind them to complete the injury forms.
