Comments:
An unannounced monitoring inspection was conducted today. The physical plant, 5 staff records, 5 children? records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. There was an abundant supply of books, toys, and materials for the children. The center was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Standard #: 22VAC40-185-150-B
Description: Based on observation, the facility failed to ensure that immunizations received were signed by a physician, their designee or an official of a local health department. Evidence: Reviewed 4 children's records and found that CH2's immunization record was not signed by a physician, their designee or an official of a local health department.

Plan of Correction: Children's immunization records were obtained.

Standard #: 22VAC40-185-60-A
Description: Based on review, the facility failed to ensure that children's records are maintained with the required information. Evidence: Reviewed 4 children's records and found that CH1 did not have documentation of verification of proof of ID. This is a repeated violation from 8/26/14

Plan of Correction: Parents were reminded to bring in documentation.

Standard #: 22VAC40-185-510-J
Description: Based on observation, the facility failed to ensure that medications are kept in a locked place using a safe locking method that prevents access by children. Evidence: Observed that there was over the counter medications in an unlocked top drawer in the "toddler" classroom.

Plan of Correction: Staff removed personal medications.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the facility failed to ensure that the most recent inspection notice was posted. Evidence: Observed that the most recent inspection notice from 8/26/14 was not posted.
