Technical Assistance:
Discussed MAT requirements for staff and activities. Discussed loft and bungee cords.

Comments:
An unannounced inspection has been completed today. The Inspector observed the Early Learning building and program. The staff to child ratios were in compliance and activities observed included outside play, lunch, and art work. The staff were friendly and answered inspectors questions. The files for staff were in order and clearances were current. Training requirements, children's records, drills, emergency preparedness plan, injury reports, and annual prevention plan were reviewed. Please call with questions 703-479-4679

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on record review of six children's files, the center has failed to obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1. Inspector randomly selected files from four age groups. Child # 2 is under 2 years of age and last immunization record is from April, 2014.

Plan of Correction: Documentation is in the file that the center has requested the information. We will have the update within the next week.

Standard #: 22VAC40-185-260-A
Description: Based on interviews and record review, the center does not have a current annual fire inspection report. The last inspection was conducted over a year ago and the permit has expired.

Plan of Correction: We have requested an inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observation of the outside playground, the center has not maintained the barrier in a safe manner due to the 4 bolts that are protruding and not flush in the barrier.

Plan of Correction: This was a result of maintenance on the playground and was corrected during the inspection.

Standard #: 22VAC40-185-330-B
Description: Based on observation, the center has failed to provide resilient surfacing under equipment with climbing apparatus to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. Evidence: 1. Two slides have been installed on a hill leading to the lower/middle playground. There was no resilient surfacing underneath and surrounding the slides.

Plan of Correction: This was corrected immediately.
