On 12/04/2014, an unannounced inspection was conducted in response to a self reported incident of children left unattended. Interviews with staff were conducted. Standard #: 22VAC40-185-340-A
Description: Based on interviews, staff failed to ensure protection and to maintain sight and sound supervision of children. Findings: On 11/24/2014, Two second grade age children left their group outside on the play area and entered the school building alone, unseen by staff, to use the restroom. Staff were unaware the two children had gone inside the building until all the children had come back inside from the outside play area and the two children were seen walking down the hall towards the SACC room.

Plan of Correction: Staff will review SACC's program wide safety and accountability system, and update the center's specific playground safety plan as needed. Staff will ensure that all children remain within sight and sound at all times.
