Technical Assistance:
Required information on written records of children's injuries was discussed with the director. A model form that may be used (optional) by the facility will be sent via email.

Comments:
A sampling of three staff records were reviewed on 4/10/2015. A mandated monitoring inspection was conducted on 4/23/2015. The inspector arrived at 9:15 AM and departed at 11:20 AM. Morning breakfast and activities were observed. medication administration and injury records were reviewed. A sampling of six children's records were reviewed. An exit meeting to review cited violations was conducted with a program director. The program director provided plans of correction and dates to be corrected during this meeting. Please contact the ispector if there are any questions.

Standard #: 22VAC40-185-280-B
Description: Based upon observation, the facility has not ensured that hazardous substances are kept in a locked place using a safe locking method that prevents access by the children. Evidence: 1. In classroom 1000E, there was a container of aerosol spray snow stored in an unlocked wall hung cabinet above the counter. The container was labeled "caution, keep out of reach of children". 2. in classroom 100H, there was a container of aerosol spray paint stored in an unlocked wall hung cabinet above the counter. The container was labeled "vapor harmful, keep out of reach of children".

Plan of Correction: Both containers were moved to cabinets with child proof locks during the inspection. The program director will speak with staff regarding hazardous substances and keeping them locked.

Standard #: 22VAC40-185-550-M
Description: Based upon review of the written records of children's injuries, the facility has not ensured that the injury records include the dates and times of the injuries and the signatures of two staff or one staff signature and a parent signature. Evidence: 1. An injury record dated 9/18/2014 for child number 2 does not include the date and time of the injury and includes only 1 signature - the signature of a staff person. 2. An injury record dated 2/13/2015 for child 2 does not include the date and time of the injury and is not signed by the parent or any staff.

Plan of Correction: The program director will ensure that the injury report is re-done to add spaces to indicate the dates and times of injuries and spaces for all required signatures.
