Comments:
A staff record review of a sampling of three staff records was conducted on 4/10/2015. A mandated monitoring inspection was conducted on 4/23/2015. On 4/23/2015, the inspector arrived at 11:50 AM and departed at 2:45 PM. Morning activities, lunch service and preparation for nap time were observed. Medication administration was reviewed. A sampling of six children's records were reviewed. An exit meeting to review cited violations was conducted with a program director. The program director provided plans of correction and dates to be corrected during this meeting. Please contact the inspector if there are any questions.

Standard #: 22VAC40-185-260-B
Description: Based upon observation, the facility has not ensured that annual approval from the health department is obtained. Evidence: The most recent documentation of health department approval is dated 11/18/13. Staff verified that documentation of a more recent inspection was not available.

Plan of Correction: Documentation of a more recent health department inspection will be obtained and faxed to the licensing office.

Standard #: 22VAC40-185-270-A
Description: Based upon observation, the facility has not ensured that areas and equipment are maintained to be in a safe and operable condition. Evidence: The areas around the bases and sides of five of the exterior windows in classroom 6 were previously taped to keep outside drafts from entering the classroom using silver duct tape. The duct tape adhesive has rotted and the tape is hanging from the windows in long pieces. As the tape is no longer around the windows, drafts from the outside are no longer blocked from entering the room. The long pieces of tape could also be entanglement hazards.

Plan of Correction: The old tape will be removed and new tape or sealant will be placed around the windows.

Standard #: 22VAC40-185-440-F
Description: Based upon observation, the facility has not ensured that rest mats are sanitized on all sides weekly or before use by another child. Evidence: The staff in the back classroom on the left were observed placing the mats out for the children's nap time. The vinyl covering on six of the mats has torn, exposing the foam inside. These torn mats cannot be adequately sanitized due to the exposed foam padding.

Plan of Correction: New mats are already on order to replace the torn mats.

Standard #: 22VAC40-191-40-D-1-C
Description: Based upon review of background checks of three staff, the facility has not ensured that criminal history reports are obtained for employees before three years since the last dates of the criminal history record checks. Evidence: 1. Documentation of the most recent criminal history record check for staff 1 is dated 3/19/2010. 2. Documentation of the most recent criminal history record check for staff 3 is dated 4/6/2010.

Plan of Correction: the criminal history record check for staff 1 was obtained and will be located. A new criminal history search for staff 3 will be ordered. A system will be put into place to ensure that documents obtained during the summer months when the office has reduced hours are placed in a secure location where they will not be filed in error..
