Comments:
An unannounced monitoring inspection was conducted on 5/14/15 from 3:15pm - 5:15pm. At the time of entrance there were 18 middle school age children in care with seven staff staff. An additional 62 school age children arrived from multiple elementary schools during the inspection. Children were observed doing homework, playing games in the gym and eating snack in the cafeteria. Records were reviewed for five children and five staff. Currently, there is no medication being stored at the facility. Emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and returned it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 22VAC40-185-500-A
Description: Based on observation, it was determined that the facility did not ensure that children wash their hands with soap and water or disposable wipes before and after eating meals or snacks. Evidence: 1. Upon arrival to the facility from the local elementary public school, at least 8 of 44 children present, proceeded to eat snack without washing their hands. 2. Staff #1 (Program Director) and the staff directly supervising the elementary school age children confirmed that some of the children did not wash their hands prior to eating.

Plan of Correction: The Unit Director will continue to train staff on correct procedures for hand washing. Current members use bay wipes, but staff will designate teen volunteers to assist in passing out wipes before and after meals. Issue will be reviewed during bi-weekly staff meetings to ensure that the procedure is effective or needs to be adjusted.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, it was determined that the facility did not ensure that the most recent inspection was posted. Evidence 1. The most inspection report was not posted anywhere in the child day center. 2. Staff #1 (Program Director) confirmed that the most recent inspection report was not posted.

Plan of Correction: The Unit Director will ensure that the most recent inspection will be posted along with the license.
