Comments:
39 children were in care during the inspection. The age range of the children was from four moths to five years old. 13 school age children arrived later. Children were observed during rest time, eating snack and free play. Three staff and five children's records reviewed. New phone number for LI 540-272-9214, Inspection time 2:20pm-4:45pm

Violations:
Standard #: 22VAC40-185-350-E-3
Description: Based on observation not all rooms are within ratio after rest time. Evidence: There were nine children in the twos room with one staff. When LI asked staff how long had the children been up from rest staff A stated 15 minutes, another staff member arrived during snack time.

Plan of Correction: We will monitoring class room and send in another staff when needed.

Standard #: 22VAC40-185-420-E-1
Description: Based on review the center failed to record tummy time. Evidence: Tummy time is not being recorded for two infants that can not roll over.

Plan of Correction: We will remind staff to record tummy time on log.

Standard #: 22VAC40-185-500-B
Description: Based on observation staff failed to use their foot to open the step on diaper pail. Evidence: staff in the infant room staff used their hands to open the foot operated diaper pail.

Plan of Correction: We will retrain the staff on diapering procedures.
