- Technical Assistance:
1. Discussed planned fall expansion and status of County approval. Center will keep VDSS abreast of progress. 2. Reviewed importance of reviewing completed background checks when received.
Conducted an unannounced monitoring inspection. Observed children + direct-care staff in the preschool-age children. Ratios and supervision were in compliance. Staff were actively engaged with the children and activities. Children were learning Spanish, and having outdoor free-play during the inspection. One class was on a field trip to a museum in Washington, DC today. Classrooms were found to be clean, safeguarded appropriately, and well supplied with educational activities and equipment for the children. Medication management was in compliance. Required documents were posted. One record keeping non-compliance was found. Inspection completed 11:05am-12:15pm.
Standard #: 22VAC40-191-40-D-1-A Description: Based on two (2) staff records reviewed, it was determined that (1) one record did not include a completed and true Sworn Disclosure Statement (SDS). Evidence: Staff #5 - Date of hire was in August 2014. The SDS signed and dated by the staff person on 7/1/14 did not disclose prior criminal history, and thus provided false information. The Criminal Record Check completed in October 2014 included evidence of a criminal conviction. Staff reported they were unaware of the discrepancies on the submitted background checks. Plan of Correction: THE DIRECTOR HAD A MEETING WITH THE STAFF MEMBER. SHE FILLED OUT A NEW SWORN DISCLOSURE STATEMENT. NEW PROCEDURES PUT IN PLACE TO PREVENT THIS IN THE FUTURE.
