Comments:
Today an unannounced renewal inspection was conducted from 2:00pm to 4:00pm. There were 54 children in care with 7 staff supervising. Ratios were maintained. Children were observed napping, playing with trains, having snack (graham crackers and water/juice), having circle time and listening to stories. Medications, first aid, 4 staff files, 4 children's files, health inspection, and the physical plant were inspected. The facility participates in a curriculum of Reggio Emilia. Children participate in long term projects using the environment and items around the facility. Standard #: 22VAC40-185-60-A
Description: Based on record review, 1 of 4 child files was incomplete. Evidence: Child #4 record did not contain documentation of original proof of identification. The facility has a copy of the birth certificate on file, but did not document when the original document was viewed and by whom.

Plan of Correction: The facility director will fill out the appropriate information on the registration form.
