Comments:
An unannounced monitoring inspection was conducted from 10:00 am - 12:15 pm. There were 25 children being supervised by 3 staff members. The children were observed in structured center play, outdoor play, and lunch. Lunch served today was macaroni and cheese, wheat bread, mixed vegetables, banana, and milk. Five staff records were reviewed. Eight children's records were reviewed and in compliance. Five medications were reviewed. There are currently three staff members certified in MAT (Medication Administration Training). The most recent fire drill was conducted 4/20/15 and the most recent shelter in place drill was conducted 9/23/14. Five accident reports were reviewed and in compliance. Violations:
Standard #: 22VAC40-185-160-C
Description: Based on review of five staff records it was determined that not all staff repeated TB (Tuberculosis) screenings every 2 years. Evidence: The most recent TB screening documented for Staff A was dated 8/22/12. This is a repeat violation. The most recent TB screening documented for Staff B was dated 3/7/2013.

Plan of Correction: Staff A and Staff B will repeat their TB screenings and submit the results to administration.

Standard #: 22VAC40-185-270-A
Description: Based on review of the school age room it was determined that not all areas of the building were maintained in safe and operable condition. Evidence: The loft located in the school age room has carpet that has become unattached from the floor and curling posing a tripping hazard.

Plan of Correction: The carpet will be repaired.

Standard #: 22VAC40-185-340-F
Description: Based on review of the two's classroom it was determined that not all children under 10 remained in sight and sound supervision. Evidence: Child A was permitted to leave the classroom to utilize the bathroom which is down the hall without adult supervision. The LI (Licensing Inspector) observed child A walking in the hallway alone returning to the classroom from the rest room.

Plan of Correction: Staff are to notify management when a child needs to leave the classroom to go to the rest room. Staff were reminded of this policy during the inspection.

Standard #: 22VAC40-185-510-E
Description: Based on review of five medications it was determined that not all medication was labeled in a manner to identify the owner, name of medication, dosage amount, and times to be given. Evidence: The emergency medication for child A did not have a prescription label on the medication indicating the name of the child, dosage amount, and times to be given.

Plan of Correction: The medication will be returned to the parent until medication with a prescription label can be provided.
