Technical Assistance:
**Discussed with staff that they were told by their superiors that they are not allowed to use tweezers on children in care, and that since they were not allowed to use them, the staff had removed the tweezers from the first aid kit. Discussed with the staff that even though they are not allowed to use the tweezers (according to their own office) they still needed to maintain tweezers on site.

Comments:
An unannounced monitoring inspection was conducted today from 10:00am-12:30pm. There were 37 school-agers directly supervised by 5 staff. The physical plant, 5 staff training records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, going outside to play and eating snack. Hand-washing procedures were also observed. There was an abundant supply of books, toys, and materials for the children. The center was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Description: Based on review, the facility failed to ensure that each child in care has documentation of a physical examination. Evidence: Reviewed 5 children's records and found that CH2 did not have documentation of a physical examination.

Description: Based on observation, the facility failed to post the most recent inspection notice. Evidence: The most recent inspection notice posted was from 2012.

Description: Based on review, the facility failed to maintain children's records with the required information. Evidence: Reviewed 5 children's records and found that CH1 and CH3 did not have documentation of addresses for both emergency contacts.
