Comments:
A monitoring inspection was conducted today from 11:45 am - 2:10pm. The physical plant, children' records, children's medications records, evacuation drills, injury reports, emergency supplies, and policies were inspected. There was an abundant supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, staff-to-child ratios were in compliance. The center was clean and organized. Standard #: 22VAC40-185-60-A
Description: Based upon record review, records were found to be missing required documentation. Evidence: Documentation of annual file reviews were missing for child.

Plan of Correction: Child's file was reviewed and documentation was made. Bi Annual review of files will be conducted to ensure that all children's files are reviewed annually. (Effective immediately).

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas of the center were found to be in need of repair. Evidence:The base trim in the rest room shared by the toddler classroom, pre- k room and the school age classroom was found to be splintered where the edges meet the doorway.

Plan of Correction: Work order was placed April 21, 2015

Standard #: 22VAC40-185-520-C
Description: Based on observation, diaper ointment was not stored in a location inaccessible to children in care. Evidence: A diaper ointment for a child who has moved to an older classroom was found in a plastic drawer in the bathroom located in the 2 year old classroom.

Plan of Correction: Diaper cream was discarded during the inspection. Preschool staff will be retrained on proper storage of diaper cream. (effective immediately).
