Standard #: 22VAC40-185-280-B
Description: Based on observation, the facility did not keep all hazardous substances in a locked place. Evidence - There were two containers of disinfecting wipes and one can of spray air freshener observed on top of the cupboards in room 115 and a carton of disinfecting wipes on top of the cupboards in the other SACC room.

Plan of Correction: Staff will lock up hazardous substance

Description: Based on observation, a SACC teacher did not provide behavioral guidance in a constructive manner intended to redirect children to appropriate behavior. Evidence - SACC teacher #1 brought a child in from the playground, placed him in a chair and told the other SACC teacher in the room that the child was to sit in the chair and was not allowed to do anything at all. This occurred after the teacher received instructions from the child's parent by telephone. The child was being punished for something that happened during the school day, not in SACC.

Plan of Correction: Staff will be trained on SACC procedures and policies

Description: Based on a review of injury reports, the facility did not provide all of the information required on injury reports. Evidence - There were not two signatures on three of the injury reports and there was no date or time an injury was reported to parents on two in jury reports.

Plan of Correction: Staff will review injury reports and sign and date
