Technical Assistance:
Consultation was given on supervision of mixed age groupings and Montessori programming for compliance with the standards.

Comments:
An unannounced renewal study was conducted. Five classes were present with direct care staff in ratio for mixed age groupings. Rooms are well equipped and maintained with materials and supplies to enhance the Montessori curriculum. The backyard area is an extension of the classroom space. Snacks are provided by a parent for each class and lunches are bought from home. Emergency medications authorizations, logs and storage were reviewed. Trainings for staff in first aid, CPR, daily health and Medication Administration Training (MAT) are current with the majority of the staff having current certification. A sample of staff and children's records were reviewed and current. Emergency preparation is also up to date. An emergency evacuation drill was conducted during the inspection. Standard #: 22VAC40-185-270-A
Description: Based on observation, one area of a classroom is not operating in a safe manner. Evidence: The closet door in classroom 5 is off its track.

Plan of Correction: The facility manager was aware of the door being off its track. Parts were ordered and he is waiting for the parts to be delivered.

Standard #: 22VAC40-185-340-A
Description: Based on observation and staff statements, a child in classroom 3 was not in sight and sound supervision. Evidence: Child #1 was on the back patio playing with a ball and observed by the Licensing Inspector. The Inspector asked the class that was on the play area if he was in their care. When the staff stated "no", the Inspector checked with the room right near where the child was playing. Both staff #1 and staff #2 in that class were involved in room activities and were not supervising child #1. Both also confirmed that the child was not wearing the "green necklace" and had not asked permission to be outside. Neither staff was aware that the child was outside and would not have been able to offer immediate assistance if required.

Plan of Correction: Had a meeting with involved staff to discuss proper protocol. Removed green necklace and restricted outdoor activities for this class for the rest of the academic year.

Standard #: 22VAC40-185-550-M
Description: Based on a sample of minor injury reports, 1 of 10 reports was incomplete. Evidence: One injury report was missing a date the parent was informed of the minor injury.

Plan of Correction: Reminded target staff about proper procedures in filling out form. Properly dated form.
