Technical Assistance:
**Discussed with the HR director that once any background check is returned with incomplete information, the facility must resubmit the background check to the necessary agency as soon as possible to complete the verification process.

Comments:
An unannounced monitoring inspection was conducted today from 12:30pm-2:39pm. There were no schoolagers in care at the moment of inspection. The physical plant, 3 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. There was an abundant supply of books, toys, and materials for the children. The center was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Standard #: 22VAC40-185-60-A
Description: Based on review, the facility failed to ensure that children's records are maintained with the required informaiton. Evidence: 1) Reviewed 5 children's records and found that CH1 was missing documentation of an address for their first emergency contact, and did not have documentation of a second emergency contact. 2) Reviewed 5 children's records and found that CH4 did not have documentation of emergency contacts. 3) Reviewed 5 children's records and found that CH5 did not have documentation of address for either of their emergency contacts. 4) Reviewed 5 children's records and found that CH2 and CH5 did not have documentation of dated emergency contact review information.

Plan of Correction: 1) Staff contacted parents of CH1 for complete emergency contact information. (5/20/15) 2) CH4's original emergency care information sheet is in his paper file. (5/15/15) 3) CH5 has complete emergency care information on his updated form.(5/15/15) 4) Both CH2 and CH5 has update emergency-care information on their updated forms. (5/14/15)

Description: Based on review, the facility failed to ensure that children's injury reports are maintained with the required information. Evidence: Reviewed 10 injury reports and found that 9 were missing documentation of the time parents were notified of the child's injury.

Plan of Correction: I informed the school nurse and extended care director that they need to ensure that they need to ensure the time of parent notification is documented at all times.
