Comments:
An unannounced renewal inspection was conducted today by the licensing inspector. The center owner/director and assistant director was present and assisted with the inspection. Most of the preschool children were on a field trip to the strawberry farm during the morning. Children were also observed during lunch and rest time. All required information was posted and a sample of staff and children's records were reviewed. Children's art work was posted in each classroom and throughout the center. The licensee participates in the USDA Food program. staff/child ratios observed: 4:23 preschool children (on the field trip- later observed having lunch and rest time) 1:4 preschool children not attending field trip. The children were coloring the letter "O" using bingo daubers. 2's 1:5 The children were coloring and looking at books. infants 1:4 The children were at the feeding table while the staff person was blowing bubbles, using a hand puppet, children were playing with infant toys. One child was drinking a bottle. toddlers 1:3 The staff person was sitting on the floor with the children on her lap, reading them books. today's menu: breakfast - nutragrain bar, orange slice, milk morning snack - celery with dip, juice lunch - chicken nuggets, broccoli, whole wheat bread, peaches, milk, mashed potatoes afternoon snack - cheeze its and juice evacuation drill 10/16/14 shelter in place - 5/28/14 &9/25/14 liability insurance expiration 11/16/15 health inspection 1/22/15 fire inspection 11/24/14 first aid/cpr - 11 staff trained health screening 5 staff trained MAT (medication) 2 staff trained plus LPN 6/30/15 Vehicle drivers have first aid/CPR training. review of emergency preparedness plan 11/2/2015 approximate time on site: 10:20 am - 3:35 pm

Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility did not maintain the center in a safe condition. Evidence: There is an open space in the wall across from the kitchen door, near the baseboard that measures approximately 1/2 inch wide and 5 inches long. It's large enough for a child to stick fingers into the open space in the wall. In the toddler room, as you enter the door, at the bottom of the wall on the immediate left, there is a piece of trim along the baseboard that has come apart creating a sharp edge.

Plan of Correction: We will have the maintenance man fix this by tomorrow.

Standard #: 22VAC40-185-510-N
Description: Based on observation, the licensee did not dispose of children's medications within 14 days. Evidence: Children #6 and #7 had medication on site that was no longer authorized for use and had not been dispensed in over 14 days. The medication should either be sent home of disposed of by the center.

Plan of Correction: Both medications will be sent home today.

Standard #: 22VAC40-185-580-A
Description: Based on inspection of the vehicle, the center did not ensure safety of children on the bus. Evidence: There were 2 tires in the back of the bus blocking the emergency exit door. Two new tires were installed on the vehicle and the old ones were placed in the vehicle.

Plan of Correction: The maintenance person will dispose of them today.
