Comments:
An unannounced monitor inspection was conducted on 5/20/2015. There were a total of eighty-nine (89) children ages 4 months ? 5 years old. Additional school age children arrived during the inspection. Complete records were reviewed during the inspection. Children were observed during arrival/departure procedures, active/quiet activities, meals/snacks, hand washing/diapering/bathroom procedures and rest/nap period. Risk Assessment and areas of non compliance found during the inspection were reviewed with the Center Director on 5/20/2015.

Violations:
Standard #: 22VAC40-185-330-B
Description: Based on inspection the Licensee failed to have sufficient amounts of resilient surfacing (mulch). EVIDENCE: On the pre-school and school age outdoor play area, equipment with moving & climbing parts did not have at least 6 inches of resilient surfacing (mulch) underneath and surrounding each piece of equipment.

Plan of Correction: Center Director recently placed an order for mulch. Additional mulch will be added to the play areas

Standard #: 22VAC40-185-370-3
Description: Based on discussion with staff, the Licensee failed to ensure daily activities for infants include outdoor time. EVIDENCE: Infant children are not taken outside daily. Staff advised LI Anderson infants are rotated throughout each week for outdoor time. Staff further advised outdoor time is not daily for each infant.

Plan of Correction: Director in the process of designing & constructing outdoor area for infants.

Standard #: 22VAC40-185-500-B
Description: Based on inspection, the Licensee failed to have diaper disposable system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. EVIDENCE: The foot-operated trash cans used to dispose pull-ups were inoperable. The foot pedals on the trash cans located in both the girls & boys rest rooms did not lift the lid when stepped on.

Plan of Correction: Director recently purchased new trash-cans & will replace them.

Standard #: 22VAC40-185-550-H
Description: Based on inspection, the Licensee failed to have required emergency information in each vehicle used to transport children. EVIDENCE: Bus #69 did not have documents containing evacuation routes potential shelters, hospitals pertaining to each site frequently visited or routes frequently driven by the center staff.

Plan of Correction: Director will update transportation book.
