A record review was completed on May 4, 2015 at the agency central office. Two staff records were reviewed. Violations related to documentation of training received by the staff were cited. Upon receipt of the inspection documentation, the licensee must develop a plan of correction for each violation. The plan of correction must include the following: ? The steps to correct noncompliance with the standard(s); ? Measures to prevent reoccurrence of noncompliance; ? Person(s) responsible for implementation and monitoring of preventive measure(s); ? Date by which noncompliance will be corrected. The licensee will have ten calendar days from receipt of the inspection documentation to complete the section titled Plan of Correction, sign each page of the documentation and return it to the Licensing Office by 5/30/2015. The licensee should retain a copy to be posted at the facility (Supplemental Information is not to be posted due to confidentiality). Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 15 calendar days, regardless of whether the Plan of Correction section is completed.

Description: Based on record review the center failed to ensure that staff records contained all required information. Two staff records were reviewed 1 - The records for staff 1and 2 do not contain documentation of any training for 2013 - 2014. 2 - The records for staff 1 and 2 do not contain documentation of the required 16 hours of 2013 - 2014. (a) Staff 1 has evidence of 10.5 training hours. (b) Staff 2 has evidence of 6 hours and 15 minutes. 3 - Staff 3 and 4, who were present during the record review, confirmed that the information was not present in the staff files.

Plan of Correction: All trainings will be documented and collected monthly by Education Manager and sent to HR for staff files. Education Manager will ensure that all training hours are met.
