Comments:
An unannounced monitoring inspection was conducted today, April 21, 2015 to determine the center's compliance with licensing standards. Upon arrival, the following ratios were observed: Head Start room #1: 4 children/ 2 staff Head Start room #2: 15 children/ 2 staff Preschool room #1: 9 children/ 1 staff Preschool room #2: 9 children/ 1 staff Preschool room #3: 10 children/ 1 staff Note: school age children had not arrived during today's inspection Children were observed playing with toys, listening to a short story read by staff, participating in arts & crafts, and washing their hands in preparation for lunch. Today?s lunch consisted of baked chicken, potato salad, green beans, tangerines, bread, and milk. All areas used by children were inspected to include the main hallways, children restrooms, classrooms, and outdoor playground. No hazards were found in these areas during today's inspection. The center had the required postings displayed in the center and on the wall upon entering the classrooms. The center's first aid kit and emergency supplies were inspected with the required supplies found. Eight children and two staff emergency records were inspected with two violations found. The following was also inspected on this day: annual fire & health inspections, emergency preparedness plan, injury prevention plan, parent agreements, liability insurance certificate, and other pertinent information. Description: Based on the inspection of staff records the required information on the reference check was not listed. Evidence: Following the inspection of two staff records, staff #1 did not have the dates in which references were checked. Licensing standards require that references be checked with the dates of contact, the person contacted, results, and signature of the person making the call. This information should be available in the staff's record for review.

Plan of Correction: Corrected on site.

Standard #: 22VAC40-185-90--A
Description: Based on the inspection of children records, the required documentation was not in the record. Evidence: Following inspection of children records, the written agreement between the center and parent was signed and in each child's record by the first date of attendance.

Plan of Correction: Per director: The parents have reviewed the agreement, however, I will have them to sign the agreement and place it in the file.
