Thank you for your time and assistance with the unannounced renewal inspection conducted on May 19, 2015. The on-site visit was from 10:30 a.m. to 3:45 p.m. On this date, at the time of arrival, there were eighty-eight (88) children in the care of fourteen (14) staff members. There were one hundred and twenty-five (125) children enrolled in the program. There were ten staff records and seven children?s records reviewed. The medication and medication paperwork were reviewed for five children. There were six staff members and four children interviewed, and six classrooms and two playgrounds were observed. The exceptions to compliance were noted on the violation notice and the risk ratings associated with those violations were noted on the supplemental page. Lunch consisted of grilled chicken breasts, macaroni salad, green beans, cantaloupe, bread, and milk. All interactions between the staff and the children seemed age appropriate and positive. Story time, art projects, outdoor play, choice time, infant feeding, infant care, diaper changing procedures, musical activities, hand washing and bathroom procedures, lunch, nap time, and dismissal procedures were observed during the visit. Standard #: 22VAC40-185-240-C Description: Based on record review and an interview with the director, one staff member had not obtained the sixteen hours of staff development/training required annually. Evidence: The records of ten staff members were reviewed during the 05/19/2015 inspection. 1. The record for Staff Member # 2 (hire/start date of 08/28/2013) contained documentation showing that 9.5 hours of annual training had been obtained between 08/01/2013 and 07/31/2014. 2. The director confirmed that Staff Member # 2 had not obtained the required 16 hours of training. Plan of Correction: If Staff Member # 2 remains employed with the center until August of 2015, she will obtain the 5.5 hours needed to complete the 16 hours required from 2013-14, as well as the 16 hours required for the 2014-15 year. Staff records and training logs will be reviewed periodically and before the end of each year to ensure that all staff members have obtained the required number of training hours.
Standard #: 22VAC40-191-40-D-1-B Description: Based on record review and verification with the director, the record for one staff member did not contain a completed criminal history check. Evidence: On 05/19/2015, ten staff members' records were reviewed. 1. There was no documentation that the center had ever received a completed criminal history record check for Staff Member # 5. 2. Date of hire for her was 11/03/2014. 3. The director found documentation where the request for the check had been sent off on 11/04/2014. She stated she had not followed up on why it had not been returned. Plan of Correction: The director will follow-up immediately with the office of the State Police to check the status of the report. She will make sure she follows-up with all background check requests to make sure they are returned within the first 30 days of employment.
Standard #: 22VAC40-191-40-D-1-C Description: Based on record review and verification with the director, one staff member's record did not contain an updated criminal history check report or an updated search of the central registry. Evidence: The records of ten staff members were reviewed during the 05/19/2015 inspection. 1. An updated criminal history check and an updated search of the central registry had not been obtained within the last three years for Staff Member # 10. 2. The date of the most recent criminal history check was 11/13/2009 and the date of the most recent search of the central registry was 06/17/2010. Plan of Correction: The director will get the requests for the two background checks sent off as soon as possible and will ensure that in the future records are checked often so that all background checks are updated every three years.
