An unannounced other inspection was conducted on 4/21/2015 & 5/14/2015. LI Anderson investigated a self-reported incident that occurred on 4/14/2014. Interviews were conducted with staff & parents relating to Standard 22VAC40-185-(5)-340-A. The information gathered during the investigation does not support the allegation, so the complaint is determined to be ?NOT VALID.? Risk assessment and areas of non-compliance found during the inspection were reviewed with the Center Director on 5/14/2015. Description: Based on record review & discussion with staff, the Licensee failed to maintain written record of children's injuries to include all required information. EVIDENCE: Written injury report for Child #1 did not include information on how the parents were notified, signatures of 1 staff 1 parent or 2 staff, future actions to prevent recurrence, treatment and names of staff that were present during the incident. LI Anderson reviewed injury report on 4/20/2015.

Plan of Correction: Center Director will review each injury report to ensure all required information is documented.
