Discussions related to varying types of mulch.
The licensing inspector conducted an unannounced monitoring inspection, at which play groups, group time, activities, outdoor time, lunch time, and pre-rest time activities were observed. Children were observed; and staff and children files and other documentation were reviewed. Thirty-three (33) children were found to be in care with 6 staff, kitchen staff, and administration. Standard #: 22VAC40-185-130-A Description: Based on review of child records, and discussions with administration, the facility failed to maintain compliance with immunization documentation as required. Evidence: Child record #2, enrollment date 4/20/15, did not contain proof of immunizations as required. Proof of immunization is required to be obtained on or by the first day of care. Plan of Correction: Proof of immunization will be obtained as soon as possible, however the child has not been in care for over 2 weeks. Immunizations will continue to be obtained prior to the childs first day in care to maintain compliance.
Standard #: 22VAC40-185-140-A Description: Based on review of child records, and discussions with administration, the facility failed to maintain compliance with child physical documentation as required. Evidence: Child record #2, enrollment date 4/20/15, did not contain a physical as required. Physicals are required to be obtained at least 30 days from enrollment, if not before. The allowed 30 days had lapsed. Plan of Correction: The physical will be obtained as soon as possible, however the child has not been in care for over 2 weeks. Physicals will continue to be obtained within the allowed 30 day time frame, if not before, to maintain compliance.
Standard #: 22VAC40-185-160-C Description: Based on review of staff records, and discussions with administration, the facility failed to maintain compliance with PPD/TB screening renewal as required. Evidence: Staff record #3 did not have a renewed PPD/TB screening as required. The one on file expired as of March 2015. PPD/TB renewals are to be completed every 2 years. Plan of Correction: This staff just returned from a 2 month leave of absence. A new PPD/TB screening will be obtained as soon as possible and placed in the file for future review. All staff will continue to receive new PPD/TB screenings every 2 years for compliance.
