Comments:
An unannounced monitoring inspection was conducted on May 28, 2015 from approximately 8:30 AM to 9:45 AM. Three staff were present with 41 children. Children were observed outside on the playground and preparing to depart to class. Three children's records were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, injury report documentation, children's records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, emergency supplies and required postings. The exit interview was conducted with the site supervisor.

Violations:
Standard #: 22VAC40-185-540-E
Description: Based on observation, the licensee did not have a working, battery-operated radio in the building. During the inspection conducted on May 28, 2015, the battery operated radio was not working.

Plan of Correction: A new radio purchases will be completed today.

Standard #: 22VAC40-185-550-D
Description: Based on record review, the licensee did not ensure practice evacuation drills were completed monthly. Evidence : The inspection conducted on May 28, 2015, there was no documentation of practice evacuation drills for the month of January and February.

Plan of Correction: The new site supervisor will make sure all drills will be completed monthly.
