Standard #: 22VAC40-185-50-A
Description: Based on observation, the facility did not treat all children's records confidentially. Evidence - There was one person present on 5/4/2015 who was not employed by SACC or volunteering for SACC who was observed getting information from a child's file.

Plan of Correction: SACC policies will be followed to ensure only SACC employees have access to children's files.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the facility did not keep all hazardous substances in a locked place. Evidence - There was a large bottle of hand sanitizer observed on the gray plastic shelf in Room #2.

Plan of Correction: Hand sanitizer will be kept in a locked place.

Standard #: 22VAC40-185-340-J
Description: Based on observation and information from staff, the facility did allow a child to leave the center unsupervised. Evidence - An 11 year old child was observed leaving the SACC center alone to ride home on a bicycle. The staff member stated that the parent had provided written permission for the child to leave the SACC center alone. The staff member stated that she had checked with her regional supervisor and was told this was acceptable.

Plan of Correction: Staff will ensure children are not allowed to leave the center unsupervised. SACC policies and licensing regulations were reviewed with all staff.

Description: Based on observation, the facility did not keep First Aid Kits inaccessible to the children. Evidence - The children help the staff by carrying First Aid Kits into the gym. While in the gym, the First Aid Kits are placed where they are accessible to children.

Plan of Correction: First aid kits will be kept inaccessible to children.

Standard #: 22VAC40-185-550-D
Description: Based on a review of the facility's emergency drill records, the facility did not conduct one drill per month. Evidence - There was no documentation of a drill being conducted in April 2015.

Plan of Correction: Monthly fire drills will be held and documented on the Emergency Drills form.

Standard #: 22VAC40-185-560-N
Description: Based on observation, the facility did not prevent one child from walking around while eating. Evidence - There was one child observed walking around while eating snack, which was cereal and milk.

Plan of Correction: Children will remain seated while eating.
