Comments:
An unannounced monitoring visit was done today, May 29,2015 beginning at 8:50 AM and ending at 3:00 PM. There were 165 children present today not including all of the school age children. The children were observed during their morning snack, circle time, free play in centers lunch and nap. The children were also observed during outside time and in their classrooms coloring and having a story read to them. Medications were reviewed as well as staff and children's files. This included a sample of newly hired and enrolled staff/children. The violations were reviewed with the director and the corrective action plan. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on observation the center failed to obtain all of the required items required for staff files. Evidence: 1. During the inspection the LI reviewed seven staff files. Five of the files were of new hires. New staff # 3 had reference checks in the record however they had not been signed and dated by the person doing the contacts. Staff #1 was in need of verification of their education as they were listed as lead teachers.

Plan of Correction: The director already told staff # 1 to bring in the verification. She will be reminded. She also corrected the reference check during the inspection.

Standard #: 22VAC40-185-330-B
Description: Based on observation the center failed to maintain the resilient surfacing on the playground at the required depth. Evidence: 1. During the inspection the LI measured the mulch on the playground and it was 2-6 " in depth in various places around the large piece of climbing equipment.

Plan of Correction: The director will have mulch delivered next week. Maintenance will be done daily on the weekly.

Standard #: 22VAC40-185-500-A
Description: Based on observation the center failed to ensure all children wash their hands prior to eating snack. Evidence: 1. During the inspection the LI observed the children eating their morning snack in the multi purpose room. One child arrived late and was given a snack at the table with the remainder of the children. This child did not wash her hands prior to eating.

Plan of Correction: The director will remind the staff to ensure all children arriving late have their hands washed prior to eating.
