Technical Assistance:
Please call the Virginia State Police to check the status on criminal history records that have not been returned within 30 days of employment. Please call the Virginia Dept. of Social Services to check the status on search of the central registry checks that have not been returned within 30 days of employment. Document the name of the representative of the agency, date agency called, status of background check, and name of person from center making the call. *Today is the 30th day of employment for one staff, and the criminal history record has not been returned to the center. ** Reminder: the double slide can not be used by children as the adjacent shed does not allow for the required six feet of fall zone.

Comments:
An unannounced monitoring inspection was conducted with center director and center staff. There were 37 children present with six staff actively supervising. The children were engaged in a variety of activities including puzzles, story time, and art. Lunch served: pizza, apple slices, cucumbers, and milk. One medication was reviewed and is in compliance. Six new staff files and three children's files were reviewed - please see Supplemental Summary for missing items. Violations:
Standard #: 22VAC40-185-150-B
Description: Based on review of three children's records, the facility failed to have immunization records that were signed by a physician, designee, or official of local health department for each child. Evidence: Child B had an immunization record in the file that had not been signed and dated by a physician.

Plan of Correction: Center staff will request the information from the parents.

Standard #: 22VAC40-185-60-A
Description: Based on review of children's files, the facility failed to have all of the required information in each child's file. Evidence: Child A did not have the name, address, and phone number of two emergency contacts. Child B and Child C did not have the name, address of phone number of one (of two) emergency contact.

Plan of Correction: Center staff will request the information from the parents.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility failed to maintain the areas outside the center in a clean condition. Evidence: there were dinner plate sized patches of powdery fungi in the mulch on the playground.

Plan of Correction: Center staff will remove the fungi from the playground.

Standard #: 22VAC40-185-430-C
Description: Based on observation, the facility failed to maintain play equipment as required. Evidence: the S-hooks used to hold the toddler swings were open more than the thickness of a penny. The S hooks were open the thickness of an index finger.

Plan of Correction: Center staff will repair or replace the S hooks on the toddler swings.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the facility failed to clean and sanitize the diapering area as required. Evidence: the two year old teacher sprayed soap and water solution and bleach and water solution on the diaper pad at the same time and wiped the pad dry. The procedure is to clean the diaper pad with soap and water and then spray with a sanitizer, such as bleach water solution. The diaper pad should be left to air dry for a minimum of at least two minutes. After two minutes, the pad can be wiped dry.

Plan of Correction: Center director will retrain staff on required diapering procedures.
