Technical Assistance:
1. Inspectors need to be able to verify that the staff present on any given day have the needed certifications/training, to include: First Aid, CPR, Medication Administration Training (MAT), and Daily Health Observation (DHO). Documentation of training and certifications should be on-site.

Comments:
Conducted an unannounced monitoring inspection. Observed 24 children + 3 direct-care staff, and 2 additional staff arrived later in the inspection. Ratios and supervision were in compliance. Interactions between staff and children were positive in nature. Children were observed arriving to the program, doing homework, having indoor free-play, washing hands, and eating snack. An art project was planned for later in the day. Supplies and equipment were sufficient. Snack included: cheese crackers and apple juice. No medications reported to be on-site. Required documents were posted. Record keeping violation found. Inspection completed 2:20-3:30pm.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on records reviewed, it was determined that there was no documentation of a required certification of the on-site staff. Evidence: There was no documentation available to identify who working today has been trained in Daily Health Observations.

Plan of Correction: A copy of the Daily Health Observation certificate for all staff will be added to the existing on-site training records.

Standard #: 22VAC40-185-270-A
Description: Based on observations made, it was determined that areas in the center were not maintained in a clean and/or safe condition. Evidence: 1. There was peeling paint on a wall located just beside the entrance to the cafeteria. Photo taken. [Photo #1] 2. Rugs were dirty with grass, dirt, paper, and string. The rugs were observed to be in this condition prior to the arrival of the children today. Photos taken of one rug, and was reflective the three rugs in the cafeteria. [Photo #2, #3, and #4.]

Plan of Correction: A work order will be submitted to school Administration to have the peeling paint area repaired/repainted. The area has been covered until repairs/repainting has been completed. 6/19/15 The rug was cleaned prior to the close of the program day. Staff were reminded of the importance of maintaining a clean environment and ensuring all cleaning takes place prior to the arrival of the children. 5/20/15
