Comments:
An announced record review was conducted on May 20, 2015. The sample size consisted of sixteen staff records. Areas of noncompliance are identified on the violation notice. Exit interview was completed with Staff A.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review, in two of sixteen staff records reviewed, the licensee did not ensure that each staff record was documented with all required information. Evidence: 1. During the inspection conducted on May 20, 2015, the records for Staff #7 and Staff #12 did not include documentation of orientation training. 2. Staff A confirmed that this documentation was not available during the inspection.

Plan of Correction: The Education Component will ensure that Human Resources receive copies of Orientation Training on start date of employees.

Standard #: 22VAC40-185-240-C
Description: Based on record review, in eleven out of sixteen staff records reviewed, the licensee did not ensure that staff who directly work with children receive 16 hours of staff development activities annually. Evidence: 1. During the inspection conducted on May 20, 2015, there was not documentation of at least 16 hours of annual training in the records for Staff #1-5, Staff #9, Staff #11 and Staff #13-16. 2. Staff A confirmed that this documentation was not available for review during the inspection.

Plan of Correction: The Education Component will ensure that 16 hours of Staff Development Activities will be conducted annually. Copies of Training logs will be forwarded to Human Resources

Standard #: 22VAC40-191-40-D-3-D
Description: Based on record review, in two of sixteen staff records reviewed, the licensee did not ensure that staff receive a search of the central registry before three years since the date of the most recent central registry finding. Evidence: 1. During the inspection conducted on May 20, 2015, the most recent search of the central registry in the record for Staff #1 was conducted on 5/02/12. 2. The most recent search of the central registry in the record for Staff #8 was conducted on 5/01/12.

Plan of Correction: #1. Central Registry Search was completed and mailed on 5/29/15. Staff #8 Central Registry Search was received on 5/26/15. The Personnel Department has implemented a tracking method of two months prior to due date for completion of Central Registry Search.
