An unannounced monitoring inspection was conducted on June 8, 2015. The director was present for the inspection. The children in care were napping in the annex building. There were 4 infants and 4 toddlers with 2 staff, and 10 preschool age children with 1 staff. It was noted that the center had purchased a 14 passenger bus to transport the children to and from school, and on field trips. Consultation was provided on lighting in the infant/toddler area during nap time. The director indicated that she was going to add another light fixture to the area to increase the lighting for added sight supervision during nap time.
Standard #: 22VAC40-185-160-C Description: Based on a review of 3 staff records, the center did not have a TB test/screening on file for a staff that had been conducted within the past two years. Evidence: The TB test/screening in the record for Staff # 1 was dated 1/31/13. Plan of Correction: The test/screening will be updated by 6/15/15.
Standard #: 22VAC40-185-440-E Description: Based on observation, the center did not ensure that there was at least 12 inches between occupied cots. Evidence: Two cots with sleeping children were placed approximately 6 inches apart. Plan of Correction: This was corrected during the inspection.
Standard #: 22VAC40-185-440-J Description: Based on observation, the center did not ensure that there was at least 12 inches between occupied cribs. Evidence: The space between 2 cribs which were occupies by sleeping infants was approximately 6 inches. Plan of Correction: This was corrected during the inspection.
