Comments:
An unannounced monitoring inspection was conducted on June 9, 2015 at approximately 11:15am - 3:40pm. Observations included indoor and outdoor areas, program, supervision, medication, health and safety, staff records and children's records. Upon entrance there were 46 children present and 10 staff members. Additional staff were on site for administrative and support purposes. Four staff records and three children's records were reviewed. Four medications and four injury reports were reviewed. The children were observed eating lunch, preparing for nap, cleaning up after lunch and nap time. Playground fall zones and resilient surfacing measured. The exit interview was conducted with the Center director.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on 2 of 4 records reviewed, the licensee did not obtain documentation of a negative tuberculosis screening with 21 days after employment. Evidence: During the inspection conducted on June , 2015, staff #2 and staff #3 (hire date 5-18-15) did not have a documentation of a tuberculosis screening.

Plan of Correction: The director reminded the staff. The staff will be sent home until they receive a T.B. test.

Standard #: 22VAC40-185-60-A
Description: Based on 1of 3 records reviewed, the licensee did not ensure all required documents for proof of child identity and age were in children's records. Evidence: During the inspection conducted on June 9, 2015, in record for child #2 there was no documentation of viewing proof of identity.

Plan of Correction: The center director with call the parent this week.

Standard #: 22VAC40-185-70-A
Description: Based on record review, the licensee did not ensure the staff records references contained all required information. Evidence: During the inspection conducted on June 9, 2015, staff #2 and staff #3 references did not have signature of the person making the reference phone calls.

Plan of Correction: The director corrected during the inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the licensee did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: During the inspection conducted on June 9, 2105, the licensing inspector observed the following in the School Age Classroom: 1-There were multiple tears on the couch exposing foam on both love seat and couch in the school age classroom. 2- There was a couch with white wooden frame board on the arm of the couch separating with exposed nails and could cause some one to be pinched. 3- In the boys bathroom near the lockers light was very dim.

Plan of Correction: The cover will be put on the couches the white wooden frame and light will be fixed today.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the licensee did not ensure all electrical outlets had protective covers. Evidence: During the inspection conducted on June 9, 2015, the Licensing Inspector observed in the 4 year old room, electrical outlet on the surge protector without protective covers.

Plan of Correction: The center director will put electrical covers on powerstrip today.

Standard #: 22VAC40-185-440-C
Description: Based on observation, the licensee did not ensure sleeping equipment was not stacked . Evidence: During the inspection conducted on June 9, 2015, the licensee inspectors observed cribs occupied each with a child in a bouncy seat.

Plan of Correction: The center director spoke to the teachers and corrected immediately.

Standard #: 22VAC40-185-440-H
Description: Based on observation and inspection of the facility, the licensee did not ensure that there was at least thirty inches of space between service sides of occupied cribs and other furniture where that space is the walkway for staff to gain access to any occupied crib. Evidence: During the inspection conducted on June 9, 2015, the licensing inspector observed 4 occupied cribs. There was approximately 12" of space between the service side of each crib.

Plan of Correction: Spoke to teachers immediately about proper spacing.

Standard #: 22VAC40-185-450-A
Description: Based on observation, the licensee did not ensure that cots and mats used by children during the designated rest period have a top cover and a bottom cover or a one-piece covering which is open on three edges. Evidence: During the inspection conducted on June 9, 2015, the licensing inspector observed in the toddlers 4 out of 9 children without a bottom cover or a one-piece covering open on three edges.

Plan of Correction: The center director will make sure that cot sheets will be available and put on the cot.

Standard #: 22VAC40-185-500-B
Description: Based on observation and staff interview, the licensee did not ensure there was a nonabsorbent surface for diapering. Evidence: During the inspection conducted on June 9, 2015, the Licensing Inspectors observed the diapering pad had multiple tears.

Plan of Correction: The center director will be replacing the cover for the changing pad.

Standard #: 22VAC40-185-550-M
Description: Based on 2 of 5 injury reports reviewed, the licensee did not ensure there was written record of the time of injury. Evidence: During the inspection conducted on June 9, 2015, the time of the injury was not documented.

Plan of Correction: The center director will speak to staff and review injury report procedures.

Standard #: 22VAC40-185-560-N
Description: Based on observation, the licensee did not ensure children were not drinking or eating while walking around. Evidence: During the inspection conducted on June 9, 2015, licensing inspector observed in the Toddler classroom a child walking around eating a roll.

Plan of Correction: The center director will speak to the staff and remind them all children need to be seated when eating.
