An unannounced inspection for license renewal was conducted today from approximately 4:00 to 5:00pm. Thirty seven children were present with five staff. Three of the staff today were substitutes. The children were engaged in free choice activities in the classroom, and playing games in the gym. Some children were practicing for their play, "Annie". Medication procedures were reviewed, and staff with current MAT, first aid and CPR training were on site. The outside play area was not checked today because of rain. Standard #: 22VAC40-185-140-A Description: Based on record review, the licensee failed to obtain documentation of a physical exam for each child before the child's attendance or within one month after attendance. Findings: At the time of the inspection, the inspector found that five children's records that had been reviewed at the 11/03/2014 inspection, still had no documentation of a physical exam. All of these children began when school started in September 2014. Plan of Correction: Staff will request the documentation of physical exams from the parents.
Standard #: 22VAC40-185-510-G Description: Based on observation, staff did not ensure that prescription medication is in the original container with pharmacy label on the container. Findings: At the time of the inspection, the inspector found a container of prescription medication for Child #1, without any prescription label on the container. Plan of Correction: The medication was returned to the parent. Staff will ensure all medications are correctly labeled.
Standard #: 22VAC40-185-540-E Description: Based on observation, staff did not ensure that a working battery operated radio was available. Findings: At the time of the inspection, the inspector found that the battery operated radio was not working. Plan of Correction: New batteries were purchased for the radio.
Standard #: 22VAC40-185-550-E Description: Based on record review, staff failed to document the dates of practice evacuation drills. Findings: At the time of the inspection, the inspector found that there was no documentation of an evacuation drill for April or May 2015. Plan of Correction: Evacuation drills will be documented immediately after they are conducted.
