Technical Assistance:
1. Electrical outlets, regardless of height on the wall, are to have protective safety covers when not in use. 2. Curled rug edges should be monitored to ensure they do not become a trip hazard. 3. All liquid spray solutions should be labeled to identify the contents. 4. The outdoor playground should be included in the tasks of being cleaned daily (litter, trash). 5. Reviewed documentation requirements for completing staff reference checks.

Comments:
Conducted an unannounced monitoring inspection. Observed 70 children + 8 direct-care staff. Ratios and supervision were in compliance. Most children were napping during the inspection, and the 5's were having outdoor play time. The infants were being held, fed, having play time on the floor, and the staff were promptly responding to their needs in a calm and nurturing manner. The center was found to be clean and sufficiently supplied with toys and equipment for the children. The cleanliness of the center has noticeably improved. Medication management was in compliance, and very organized. Written injury reports were in compliance. Required documents were posted. Record keeping was organized, though violations found in staff records. Inspection completed 1:00-2:50pm.

Description: Based on 2 staff records reviewed, it has been determined that 1 record did not include a TB test. Evidence: Staff #13 - Date of hire was July 2014, and the 21-day grace period has expired.

Standard #: 22VAC40-185-280-B
Description: Based on observations made, it was determined that hazardous substances such as cleaning supplies and pesticides were not kept in a locked place. Evidence: (1) 3 spray cleaning products were found in an unlocked upper level cabinet in the 5's/School-age room. (2) Multiple cleaning products, laundry products, and pesticide products were found to be in an unlocked hall storage closet.

Description: Based on observations made, it was determined that occupied cots in one room did not have a minimum of 12 inches of space between them. Evidence: 7 of 15 cots had a varied space of approximately 6" to touching.
