22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted beginning at 10:40 a.m. and ending at 1:30 p.m. with the assistant director. There were thirty-seven children in care with seven staff. Infants were observed finishing snack and playing on the floor. Children were observed having playtime on the playground. The school aged children were on a field trip. Areas of the center were observed. The playground was inspected. Children were observed lining up to wash their hands before lunch and having lunch. Lunch was fish sticks, mandarin oranges, mixed vegetables and milk. Five children's records and three staff records were reviewed. Two medications, authorizations and logs were reviewed. Standard #: 22VAC40-185-60-A
Description: Based on review of children's records, not all records were complete. Evidence: Child A, enrolled 5/21/2015, did not have two full addresses for emergency contacts.

Plan of Correction: Parent has updated child's file with the missing addresses.

Standard #: 22VAC40-185-270-A
Description: Based on observation, not all areas of the center and equipment were maintained in clean, safe and operable condition. Evidence: There is a large crack in the ceiling of the upstairs bathroom. The paint and plaster are chipped, broken and falling. A light fixture in the downstairs bathroom was covered in thick dust. The hot water was not operable in the first sink in the downstairs bathroom. A toy on the toddler playground was broken, exposing the springs and small pieces in the battery port, posing a choking hazard.

Plan of Correction: All light fixtures have been dusted and cleaned. Hot water was restored to sinks. The toy was removed during inspection.

Standard #: 22VAC40-185-280-B
Description: Based on observation, not all hazardous substances were kept in a locked place as required. Evidence: In the infant room, bleach solution was observed on a high shelf. In the toddler classroom, bleach solution was observed on a shelf. Active diapering was not being performed in any of the classrooms. In the two's classroom, Chase brand fake snow was observed on a middle shelf accessible to children. The fake snow was labeled "harmful if ingested" and "keep out of reach of children."

Plan of Correction: All hazardous substances were placed in a locked closet during the inspection. Fake snow was thrown in the trash.

Standard #: 22VAC40-185-330-B
Description: Based on observation, the resilient surfacing on the playground was not the required depth. Evidence: The shredded rubber mulch was measured two inches under the yellow metal climbing structure ("monkey bar ladder") and four to five inches around the see saw. The shredded rubber mulch is required to be at least 6 inches in depth. There was mud and dirt observed on the toddler playground under the Little Tikes climbing structure and the seesaw. The mulch is required to be at least six inches under climbing structures or moveable equipment.

Plan of Correction: Added mulch to all play structures.

Description: Based on observation, not all diapering ointments were labeled as required. Evidence: In the two's classroom, there was an A&D ointment and Desitin ointment that were not labeled with the child's name.

Plan of Correction: Diapering ointment was thrown in the trash

Standard #: 22VAC40-185-570-E
Description: Based on observation, not all infant bottles were dated as required. Evidence: An older infant's bottle was not dated.

Plan of Correction: All bottles were dated.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the findings from the most recent inspection were not posted as required. Evidence: The most recent inspection documents posted were from 12/18/2014. There had been two subsequent inspections on 2/20/2015 and 3/17/2015.
