An unannounced monitoring inspection was conducted on 7/2/15 from 10:25am - 11:50am. During the inspection there were 31 children ages 5 12 years old in care with five staff. Upon arrival to the facility, the children were observed returning from a field trip. Once in the facility, children were observed playing games, reading books and various other activities in the classrooms. Records were reviewed for five children and five staff. Medication, emergency procedures and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).
Standard #: 22VAC40-185-270-A Description: Based on observation, it was determined that the facility did not ensure that all areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition. Evidence: 1. There was a clear plastic container on the playground that had multiple areas where the plastic was cracked and could pinch a child's fingers. 2. The blue plastic ball section return game located on the playground was broken at point where it attaches to the metal pole. It is currently down on the ground with the metal pole sticking out of it. The piece that has fall down is not secure and could injure a child. In addition, there are areas where the plastic is cracked and a child could pinch their fingers. Plan of Correction: The clear, plastic container was removed during the inspection. The blue plastic portion for the ball return game will be repaired or removed. Until, the broken section of the ball return game is repaired or removed, staff will keep children away from it. Staff will check the playground for nay broken equipment and remove it immediately.
