Comments:
A focused investigation was conducted today in response to a self-reported incident received by our office regarding forbidden actions. Staff records were reviewed and multiple staff were interviewed. Based upon staff interviews and video footage reviewed the preponderance of evidence supports a violation. Violations:
Standard #: 22VAC40-185-410-1
Description: Based on staff interviews and video camera footage, a staff member participated in a forbidden action of roughly handling a child of 23 months. Evidence: On April 28th, 2015 at approximately 4:00pm, during snack time, Staff 1 was observed walking Child 1 back to the snack table by the child's wrist. Child 1 fell away from Staff 1 and Staff 1 held the child's arm and pulled back on it causing a wrist injury. Child 1was not using her hands during "itsy bitsy spider" and was not moving it during the afternoon. Staff 2 brought it to the attention of the director.

Plan of Correction: Upon notification of the incident occurring in our building, the director team reviewed the cameras to watch the incident on the date and time at which it occurred. After reviewing the tapes. both staff members who were involved and witnessed the incident were immediately terminated and escorted out of the building. The director team then notified the office of licensing as well as CPS to inform them of the incident and the actions we had taken. The child's parents were also notified immediately and informed of the incident and the resulting actions.
