Thank you for your time and assistance during the unannounced monitoring inspection conducted on July 1, 2015, between the hours of approximately 10:15 a.m. and 4:35 p.m. Forty-three (43) children were enrolled and twenty-three (23) children were in the care of eight staff members during the on site inspection. Six children's records and twelve (12) staff records were reviewed and/or updated. Seven children's medications, accompanying authorization forms and medication logs were reviewed. Safety equipment (first aid kits and flashlights ), posted information (license, emergency plan diagrams, emergency telephone numbers and classroom schedules) and available documents (emergency evacuation drill record, field trip permission forms, swimming permission forms and injury report forms) were observed/reviewed. The daily schedule accurately reflected the activities provided. All food was provided by the children's parents. Observed staff/child interactions were positive and supportive of children's growth and development. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Program observation was completed. Activities observed on this date included: indoor and outdoor play; infant care; stories and songs; and children's return from a field trip. Also observed were restroom and handwashing procedures; lunch; naptime; and departure/pick-up of children. Exceptions to program compliance were noted on the violation notice. Standard #: 22VAC40-185-530-A Description: Based on observation, staff record review/update and discussion with staff on July 1, 2015, the program failed to ensure a person currently certified in first aid, cardiopulmonary resuscitation (CPR) and rescue breathing accompanied the children on a field trip to the movies, on this date. Evidence: 1. When the licensing inspector (L.I.) arrived at the facility at 10:15 a.m. on this date, the school age children were on a field trip. The L.I. was told by staff #9 that eight children and two staff members (staff #7 and staff #12) had gone to the movies. 2. Review/update of staff records, on this date, revealed that first aid/CPR certification for staff members #7 and #12 had expired on June 30, 2015. 3. Discussion with staff #7 confirmed that first aid/CPR certification had expired for the two staff members who took the children to the movies. Plan of Correction: A first aid/CPR class has been scheduled for July 13, 2015. In the future, at least one staff member currently certified in first aid, CPR and rescue breathing will accompany children on field trips.
Standard #: 22VAC40-185-550-M Description: Based on review of injury report forms and discussion with staff #7 on July 1, 2015, program staff failed to ensure the records consistently included documentation of the date, time and/or method of parent notification of minor injuries. Evidence: 1. Review of injury report forms, on this date, determined that information regarding the date, time and/or method of parent notification of minor injuries was not consistently documented on the forms. 2. Staff #7 verified that staff members completing the injury report forms did not consistently document the parent notification information. Plan of Correction: Completion of injury report forms has been reviewed with all staff members. Administrative staff will review the injury report forms each month, to ensure staff members are documenting all required information.
