22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted today, July 16, 2015. The violation from the previous inspection has been corrected. A violation notice was completed regarding the violations found during today's inspection. Upon arrival to the center the inspectors observed the classrooms. The school age class was busy preparing for a field trip. The other classes went outside to the playgrounds. Lunch was observed which consisted of chicken sandwich, peas, pineapple chunks and milk. Should you have any questions please do not hesitate to call. Inspection began: 9:35 am and ended approximately 12:15 pm.

Standard #: 22VAC40-185-270-A
Description: Based on observation of the building the center failed to maintain some areas in a safe and operable condition. Evidence: 1. Located in the wobblers room the windows have handles that roll out the windows. Several of these handles are broken off and have sharp edges. This area is accessible to the children. 2. The stationary climbing equipment on the playground has a walking bridge. This bridge is covered with a plastic substance that is peeling off due to weather and age. The exposed area is now rusting and has sharp edges and is accessible to the children. 3. Located on the side playground are two metal pieces that have a wooden seat and three steering wheels attached. The metal piece is rusting.

Plan of Correction: A work order has been placed requesting that the window handles be replaced. The director will contact the manufacturer to see of there is a way to replace the walking bridge or how to make repairs. The metal pieces will be removed from the playground and painted to cover the rusting areas.

Description: Based on observation of the play equipment the center did not ensure that all equipment meets requirements. Evidence: 1. The tire swing has S-hooks that have openings greater than a penny.

Plan of Correction: A request will be made for the maintenance to close the S-hooks. Staff will monitor this area on a weekly basis.

Description: Based on review of the medications located at the center, the form used did not have all the required information. Evidence: 1. The form signed by the parent did not have the name of the medication, dosage amount and date to end medication.

Plan of Correction: The medication administration permission form has been completed by the parent. Staff will ensure that when accepting medication they will have the parents complete all the information on the form.

Standard #: 22VAC40-185-510-J
Description: Based on review of medications in the facility it was found that one medication was not locked. Evidence: 1. Located in the refrigerator in the infant room is a prescription medication. This medication is not locked.

Plan of Correction: The medication was moved to the locked box in the refrigerator. A safety lock will be placed on the refrigerator in the infant room.
