Discussion was held on the topics of current enrollment and current staffing.
The following inspection took place between the hours of 11 am through 1:30 pm. Today, I met with the Staff of the Ashburn Village Sports Pavilion located in Ashburn, Virginia. At the start of this unannounced monitoring inspection there were a total of 67 children with 8 staff; good ratios in all areas. A total of 3 children?s files and 8 staff files were reviewed. The children were engaged in the following: games, outdoor swim time, art, large motor skills in the gym, bathroom breaks, hand washing, lunch, and other organized activities. Standard #: 22VAC40-185-160-A Description: Based on review and interview, 1 of 8 staff files reviewed indicated that each staff member did not submit documentation of a negative tuberculosis screening (TB) no later than 21 days after employment. Evidence - 1. On the date of inspection (07/16/2015), there was no TB documentation on file for Staff #8. 2. The Director stated (during an interview conducted on the date of inspection) that the start date for Staff #8 was 06/16/2015. Plan of Correction: Staff #8 will be informed of this missing documentation and will be required to submit it to the center.
Standard #: 22VAC40-185-160-C Description: Based on review, 2 of 8 staff files reviewed indicated that the tuberculosis (TB) documentation on file was more than two years from the date of the most recent screening or testing. Evidence - 1. On the date of inspection (07/16/2015), the most recent TB documentation on file for Staff #1 was dated - 06/14/2013. 2. The most recent TB documentation on file for Staff #3 was dated - 06/11/2013. Plan of Correction: Updated TB documentation will be obtained and submitted for both Staff #1, and Staff #3.
Standard #: 22VAC40-185-560-G Description: Based on observation, food that was brought from home was not dated. Plan of Correction: An allowable variance will be considered. In the meantime, staff will hand out dated stickers to mark each lunch container for every child on a daily basis.
