The following standards were reviewed: The center shall maintain (i) written permission from the parent of each child who participates in swimming or wading and (ii) a statement from the parent advising of a child's swimming skills before the child is allowed in water above the child's shoulder height. If sunscreen is used, the following requirements shall be met: Written parent authorization noting any known adverse reactions shall be obtained.
An unannounced renewal inspection was conducted from 10:00 A.M. until 12:00 P.M. At the time of the inspection, 40 children were in care with two staff members and the cook present. The sample size consisted of three children's records, all staff records, and one child's medication. Children and staff were observed engaged in free indoor and outdoor play and swimming. Staff were observed during transporting children to the pool, transition, and behavioral management. During free time children played pool, karaoke, basketball, computers, video games, reading, and socializing . Violations from the previous inspection were reviewed for current compliance. No repeat violations were found. Areas of noncompliance are listed on the violation notice.
Standard #: 22VAC40-185-480-B Description: Based on record review and interview, the director failed to obtain written permission from the parent of each child who participates in swimming and a statement from the parent advising of the children's swimming skills before the children were allowed in the water above the children's shoulder height. Evidence: 1. Out of the three children's records reviewed none had written permission from the parent allowing children to swim or advising of swimming skill level. 2. The director stated he recently got written permission from some parents but not all. 3. All 40 children in attendance went to the public park pool to swim today. Plan of Correction: The parents who have not returned the swimming permission slip will be given a new form to sign. The director will follow-up to ensure all parents complete the form.
Standard #: 22VAC40-185-520-B Description: Based on record review and interview, the administration failed to obtain written parent permission for sunscreen with any adverse reactions listed. Evidence: 1. The three children's records reviewed did not contain documentation regarding sunblock. 2. The director stated they did not give parents a form regarding sunblock to sign. Plan of Correction: All parents will be given a parent permission form for sunscreen with a request for any adverse reactions to sunscreen to be listed on the document. The director will follow-up to ensure all parents complete the form.
