Technical Assistance:

The licensing inspector conducted an unannounced monitoring inspection July 23, 2015 from approximately 10:10am to 12:45pm. The following ratios were observed with a total of 47 children present during the inspection: Infants - 4 children with 1 staff Toddlers - 5 children with 1 staff Twos - 6 children with 1 staff Threes? 7 children with 1 staff Fours & Fives - 11 children with 2 staff School Age - 14 children with 1 staff During the inspection, the Infants were observed sleeping in their cribs and playing on the floor with toys. The Toddlers were observed dancing, washing their hands and eating lunch. The Twos were observed eating lunch. The Threes classroom was observed looking at books, taking bathroom breaks, creating a square collage, and washing their hands for lunch. The Fours and Fives were singing songs and eating lunch. The school age children were observed selecting the star student of the day, creating Hawiaan headbands, and making a coconut tree. Lunch consisted of corn dog nuggets, green beans, pineapples and milk. Later children began to transition into naptime. All areas of the center including classrooms, hallways, bathrooms, kitchen, and playgrounds were inspected. The center was equipped with toys and supplies and items were available to the children. The following information was reviewed: emergency supplies, injury reports, fire and shelter-in-place drills, liability insurance policy, menu, daily attendance, daily sheets, parent agreements, and staff training information. The owner stated medication is not administered at this time. Transportation is provided. The van and transportation documents were inspected. Last documented health inspection: 03/17/15 Last documented fire inspection: 07/29/14 During the inspection, five children records and four staff records were reviewed. Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not ensure all areas were kept in a safe and operable condition. Evidence: During an inspection of the Toddler playground, the licensing inspector observed an approximate four foot by four foot square wooded box located in the middle of the playground. The wooden box had peeling paint and the wood was rotting and the corners were splintered creating a hazard that could entangle clothing or snag skin. The staff members from the Toddler and Twos classrooms stated the children have been using the Toddler playground.

Plan of Correction: Per owner - We will not allow the children back on the Toddler program until the playground is fixed.

Description: Based on observation and interview, one spray bottle did not indicate its contents in one of six classrooms inspected. Evidence: During an inspection of the Toddler's classroom, the licensing inspector observed a spray bottle containing a clear liquid located in a locked cabinet. The spray bottle was not labeled in a way that indicated its contents. A staff member in the Toddler's classroom stated the spray bottle contained bleach water and the bottle was new.

Plan of Correction: Per owner - We will label the spray bottle today.

Description: Based on observation and measurements, the center did not maintain the required depth of resilient surfacing in all fall zone areas. Evidence: During an inspection of the playgrounds, the licensing inspector observed the following: 1. The mulch at the end of the blue little tykes slide (approximately two and a half feet tall) located on the Toddler playground measured three inches in depth. 2. The green slide (approximately two feet tall) located on the Toddler playground was sitting on a dirt surface. 3. The swings located on the Toddler playground were located over a dirt surface. 4. The pea gravel at the end of the pink Fisher and Price slide (approximately three feet tall) located on the Preschool measured approximately four inches in depth. Per owner - We will not allow the children back on the Toddler program until the playground is fixed.

Plan of Correction: Per owner - the children will not be allowed to use the Toddler playground until additional mulch is added. The pea gravel on the Preschool playground will be redistributed to ensure there is an adequate amount at the end of the slide.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center did not ensure the diaper changing surface was non-absorbent. Evidence: During the inspection, the licensing inspector observed a diaper changing pad with tears in it located on the diaper changing table in the Infant room.

Plan of Correction: Per owner - I will buy a new diaper changing pad today to replace the torn one.
